Villa Madonna Academy Registration Form 
 

Thank you for choosing Villa for the 2023-2024 school year. Please complete this form and submit a $400 registration deposit per child within five business days. The $400 per student is nonrefundable, nontransferable, and will be applied to tuition. However, if you apply for scholarships and/or tuition assistance, $300 of the $400 deposit will be refunded if the student chooses to not attend Villa and advises Villa of the intention to not attend within 30 days after receiving the tuition assistance or scholarship award.

 
Only one form submission per family is required.
 
Please note that there are four steps to this registration process: 
  1. Complete the Financial Responsibility Form below.
  2. Pay the $400 registration deposit. 
  3. Register with FACTS. All families are required to have a FACTS account this year. If you are a returning Villa family, and already have an account, your family information will be transferred to the 2023-2024 year and you will be assessed a $45 service fee. If you have opted for the FACTS $20 "Peace of Mind Insurance", it will be added to the $45 fee. 
  4. Apply for scholarships and tuition assistance if applicable.
